For Assisted Living and Memory Support residents, all residents are given a comprehensive assessment to determine the appropriate care plan. These assessments guide our care partners when establishing individualized services. We empower residents by focusing on their preferences and personalized needs. Our care partners provide support and care with compassion and integrity while helping to foster independence, and our holistic approach to person-centered wellness allows residents to easily and comfortably age in place. Our services are provided under the supervision of a Health Service Director. Our Health Service Director performs the following services:
- Supervises staff and care partners in administering services and care to residents
- Communicates and coordinates with resident health care providers
- Conducts resident assessments as needed in order to meet resident’s changing care needs
- Oversees resident’s needs, including ensuring resident receives proper activities of daily living support and care
- Schedules and participates as needed in resident and family care conferences
